Prior to your examination, your benefits can be confirmed to allow the most accurate and responsible planning of your care. Once you provide your information such as insurance company, policy number, name and DOB, our office will contact your carrier to get a breakdown of your coverage including any copayments, outstanding deductible balance, and the number of visits allowed in your plan per year. The following insurance is currently accepted at Holistic Physical Therapy, Inc.:

In some cases, out-of-network benefits can be used with our clinic. Calling our office and having your benefits reviewed is the best way to determine if this is a good option.
Additionally, private pay is also accepted. The rate for the initial evaluation is $100 and $70 for each follow-up session.
The duration and frequency of treatment sessions varies based on each individual plan of care. It is important to disclose any specific parameters you wish to follow at the time of your first appointment.

Frequently Asked Questions
Do I need a referral to receive physical therapy?
No. You do not need a referral or prescription to begin physical therapy treatment.

Do I need insurance to receive physical therapy?
No, our cash payment rate is $120 for the initial evaluation and $80 for each follow-up visit.

What is direct access?
In California, patient's can be seen for physical therapy treatment without a referral for a term of 12 visits or 45 days, whichever comes first. It will be necessary to get a referral after this time frame should treatment need to extend for a longer time.
Medicare is a current exception to this rule with a unique time frame for getting a referral, which can be discussed with our clinician.
What if I cannot afford to attend therapy as often as recommended by the physical therapist?
Any limitations in availability and/or finances should be discussed openly and honestly with the therapist. Mutual agreement on the scheduling and nature of your care is essential for success. Accommodations can be made in the form of decrease frequency of therapy and increased volume of exercise/self-care performed at home.

Will I have to pay for physical therapy treatment in office?
Payment for treatment will be due at the time of service per our office policy. Payments may be related to unmet deductible amounts, copayments, or patient balances.

What forms of payment is accepted?
All major credit cards, debit, check, and cash.
Venmo and Zelle are not currently accepted.
